    If my application could be taken into favorable consideration, I would be
most grateful.
    I hope you will kindly send me an application(申请单) form.
    I would greatly appreciate receiving copies of the required application
forms(申请表格) and any instructions(说明) you may be so kind to give me regarding my
possible admission.
    Enclosed(附上) are my score reports on all subjects and
tworecommendations(推荐信).
